Position Overview

Are you ready to make a difference in the lives of others? We are seeking a passionate Occupational Therapist (OT) to join our dynamic team! In this role, you will leverage your clinical expertise to assess, teach, and deliver therapy interventions as directed by the Physician, all while following the home health plan of care.

Schedule: Enjoy a balanced work-life with Monday to Friday visits, totaling 30+ hours a week, along with full-time benefits.

Location: Serve the beautiful communities of Mauston, WI Dells, Hillsboro, and Necedah.

Essential Job Functions

• Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current levels of function.
• Administer, evaluate, and interpret tests and measurements to set functional goals and outcomes.
• Develop a comprehensive written evaluation and plan of care that outlines occupational therapy needs, goals, progress, and expected outcomes.
• Deliver occupational therapy services in accordance with prescribed physician orders.
• Maintain the plan of care through timely documentation of client performance, adjusting goals and treatment as necessary, and determining appropriate discharge care plans.
• Lead discharge planning, collaborating with and educating patients and their caretakers on home exercise plans post-discharge.
• Coordinate services with the healthcare team, including physicians, parents/guardians, and nurses.
• Document according to policy and procedure with minimal supervision, ensuring effective communication and documentation to avoid technical or clinical denials.
• Serve as a resource consultant for clinical staff, maintaining effective communication with team members and families.
• Supervise other personnel (OTA, HHA) as applicable.
• Complete OASIS assessments where permitted by state regulations.

Requirements

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR certification.

Preferred Qualifications

• Strong organizational and communication skills.
• Attention to detail and effective time management.
• Proven ability to build relationships.
• Experience in Home Health Therapy and familiarity with OASIS documentation.
